All senior raters must also give an overall performance evaluation. Call me a nitpick, but the check boxes on the second page 216-8 even line up the same. I am constantly having problems where depending upon the selection from the drop down menu, it doesn’t allow me to actually type in the block. To fix it, I have to copy and paste what I already have in there if anything and re-do the drop down block to say pass.
Boxes B and C require the information from any senior raters or reviewer who is also submitting the evaluation 21668- for the NCO. Here is the link: Which subsequently blanks the entire box, but at least allows me to type for a short period of time.

Horrible form but those who created it probably don’t have to deal with the process. The APFT block locks down to editing or entering bullets.
Part I requires the basic information about the NCO that is to be evaluated. The five areas are competence, physical fitness, leadership, training, and responsibility and accountability.
